SHAWNEE, OKLA. – Employees and teams from home health, hospice and Medicaid ADvantage agencies across the state of Oklahoma were honored today at A Path of Care’s annual “Light the Path” 2024 Awards ceremony held in Shawnee. Awards were presented to employees and agencies for their superior service and accomplishments in 2023. 

A Path of Care is a statewide network of Oklahoma-owned agencies providing different types of care to patients in their homes and communities. The awards presentation took place at Gordon Cooper Technology Center.

“In 2023, our network touched the lives of more than 5,300 Oklahomans,” explained A Path of Care CEO Brian F. Wilson, R.Ph. “We launched our annual Light the Path awards to honor those within the network who excel in caring for the whole person, not just an illness. We strive to help heal the body, mind, spirit, and soul to position Oklahomans for a healthy life now and in the future.”

The Light the Path awards included several categories including:

Individual Awards

  • Extraordinaire Awards for the employee or office that has done something extraordinary to have an impact on the path of others.
  • PLUS Awards (Performance, Leadership, Understanding and Respect) for the person who exemplifies and demonstrates these standards in action, day in and day out.
  • ABC (Above and Beyond the Call) Awards for the person or office that always jumps in to help others without complaint, routinely going above and beyond in caring for patients and helping coworkers during challenging times.
  • Outstanding Clinician Awards recognizing clinicians who blend outstanding clinical skills with compassion.
  • Outstanding Aide Award for those who are capable, dependable and compassionate in helping others live independently in their homes.
  • Outstanding Therapist Awards to therapists who blend knowledge and skill with outstanding communication.
  • Star Performer Awards recognizing members of the marketing team who represent their organizations professionally in the community and serve as trusted team members in their agencies.

Team Awards

  • Team Spirit Award for the team with the highest staff retention.  
  • Patient Experience Awards to the teams with the highest “Most Likely to Recommend” scores in patient/family surveys.
  • Financial Foundation Award to the team with the best billing turnaround and results for strong financial stability.
  • In Sync Award to the team with the fastest average sync time to make patient care information available to others on the team.
  • Overall Growth Award to the team with the highest growth in census in 2023.
  • All Star Awards to the teams with the highest or most consistently high Star Ratings in Home Health.
  • High Five Award to the team with the best overall rankings in five categories: Census, Value-Based Purchasing, Star Ratings, Patient Surveys and Financials.
